IVF Success & Azoospermic Male Patients

IVF Success with Azoospermia:

Azoospermia commonly referred to as “no sperm count” is a male fertility issue many men face. Azoospermia is defined as the complete lack of sperm in the ejaculate. It occurs in 5% of infertile men. TESA with IVF/ICSI has successfully treated azoospermia and offers approximately a 40% live birth rate. These techniques offer the possibility of several attempts of IVF/ICSI from a single testicular biopsy sample.

Obstructive Azoospermia Treatment via IVF:

Obstructive Azoospermia occurs when sperm is being produced in the testes but cannot be found in the semen because of a blockage in the male reproductive tract. The good news is that in most cases of Obstructive Azoospermia, sperm can be recovered through microsurgery and used in an IVF-ICSI treatment.

Conditions That Causes Azoospermia:

  • Genetic infertility due to abnormal chromosomes (karyotype)
  • Unexplained genetic infertility
  • Cancer treatment (chemotherapy, radiation, surgery)
  • Varicocele effect
  • Pituitary suppression, drug induced (anabolic steroids, alcohol, glucocorticoids)
  • Testosterone supplements
  • Severe illness (cancer, kidney or liver failure)
  • Diabetes mellitus
  • Pesticide/toxin exposure (including hot tubs and baths)
  • Undescended testicles at birth
  • Ejaculatory duct obstruction
  • Vasectomy

Some Factors Effecting IVF Success Rates

Among all the assisted reproductive technologies (ART) available, IVF and ICSI have the best success rates. Although IVF is a complex process and exact outcome of a couple cannot be predicted however, few of the factors that can improve the results are as follow:

  • Age:

IVF success rate decline with an increase in age and for this reason early intervention is advised.

  • Infertility duration:

It is seen that there is decrease in outcome results with increased duration of infertility

  • Cause of infertility:

The positive outcome of IVF increases in cases of tubal pathology, endometriosis, ovulation failure and male factor due to decreased sperm count or immotile sperms

  • Life style modifications:

Smoking and obesity are considered risk factors for infertility for both male and female. Quitting smoking and losing weight can help increase the result of IVF treatment

Endometriosis and Infertility

Endometriosis is the presence of endometrial tissue outside the uterus resulting in an inflammatory response in the body. This inflammation can cause scarring, adhesion and an inhospitable environment for the egg causing infertility.

Common symptoms include pain during menstruation, intercourse, bowel movements and urination. Excessive bleeding can also be present. Infertility is a common complication of endometriosis. Diagnosis is made after taking detailed history, ultrasound and visualizing the endometrial tissue by laparoscopy.

The treatment of endometriosis depends on the severity of disease. But for fertility treatment of such patients assisted reproductive technologies like artificial insemination and In-Vitro Fertilization (IVF) can be used. However if pelvic anatomy is disturbed and the endometriosis is severe then IUI treatment success becomes unlikely and IVF becomes the treatment of choice.

In IVF procedure egg is removed from ovary, fertilized in a test tube (In Vitro) and embryo is transferred into uterus bypassing the scarred adhesive tissue resulting in better chances of a positive pregnancy.
